Northern Ireland End of Life Care in Hospital: Experience Survey

We would like to offer our sincere condolences to you and your family as we are aware that someone close to you has died recently in hospital.

You have received this link to this survey because we would like to know about your experience of the care you and your loved one received on their final admission to hospital. Your experiences will inform a regional report to continue to improve the support and care offered to our patients and those important to them.

We know that things have been different this year due to COVID-19 and you may not have been able to be with the person or involved in their care as would have been possible prior to the pandemic. Some of the questions may therefore be more difficult to answer.

It is your choice if you would like to complete the survey. It is possible that this survey may bring back memories or emotions associated with your recent loss. If you feel upset or distressed you do not have to continue with the survey, or can stop and complete it later. We have included a list of bereavement support organisations in the related documents section below if you would like to talk about how you are feeling.

Completing the survey:

  • The survey should take around 15 minutes to complete.
  • Most questions will be answered by selecting one box that best reflects your experience.
  • At the end of the survey, there is an opportunity to add further comments. Please do not write your name, address or add any information that could identify you.
  • The closing date for this survey is 31st October 2021.
  • Please note, this survey is separate from the hospital’s complaints process. If you want to raise a concern or complaint with the service provided or get involved with helping shape future services, please contact the relevant Health and Social Care Trust details of which can be found in the related documents section below.

The information you provide in the survey will be anonymous and kept confidential.  Please do not write your name, address or add any information that could identify you anywhere on this survey.  

How we will use your information:

By completing this survey, you are permitting us to the use of the information you provide in a report which will inform the care of patients in the future. Reporting will be carried out in such a way that your individual answers will not be identifiable. Any additional comments you add may be quoted anonymously.
