Helping the Inquiry

The Inquiry is now preparing for a phase of evidence dealing with staff and management at Muckamore Abbey Hospital. The Inquiry wishes to hear from current and former members of staff who have information that will assist the Inquiry panel in addressing the terms of reference.

Making Contact

The Inquiry would like you to provide your contact details and answer some basic questions. Once you do this, we will be in contact with you about the next steps.

The Inquiry wants to obtain these basic details as quickly as possible. This will enable the Inquiry to put the necessary arrangements in place to take statements where appropriate. The Chair would be very grateful if you could submit your contact form as soon as possible and in any case by Thursday 30 November 2023.

In certain circumstances the Chair can grant anonymity to witnesses and you should indicate below if you think that may apply to you.
