Your Experience of the Multi-Disciplinary Team (MDT) at your GP Surgery (Causeway Federation)

Overview

10,000 MORE Voices collects stories from patients, families and carers so that
we can understand your experience and make sure that your contact with our
services is as good as possible.

If you have had contact with any member of the Multi-Disciplinary Team (MDT)
as part of your GP Surgery in the last year we would like to hear your story. This
survey does not cover experiences with your GP.

We want to hear about:

• First Contact Physiotherapist
• Social Worker
• Social Work Assistant
• Mental Health Practitioner

We would really like to understand if the Multi-Disciplinary teams helped you to
get the right care at the right time and if they gave you the advice and support that you needed. If you needed help from other services, did the MDTs help you to access those other services?

All surveys are anonymous; you do not need to give your name and we ask that
you do not share names of the patient, carers or staff within your story. The story that you provide will be used to improve our services through written reports and staff education and training.
