This form should be completed for;

• all trailers not appearing on the booking system
• all other vehicles not yet registered in the United Kingdom (UK)

Please complete all questions in the survey and allow 2 clear working days before booking a vehicle test.

Vehicles already registered in the UK but not showing on the booking system can be added here. You can continue to book an appointment when the vehicle has been added.
