Overview
Thank you for being part of the Serious Adverse Incident (SAI) Review.
We welcome your time in providing feedback on your involvement and how we have engaged with you.
The evidence gathered from responses will aid us in improving our services.
The survey should take no more than 10 minutes to complete. Responses will be reported in an aggregated and anonymised form.
To ensure the integrity of the survey, participants are asked to provide an email address for validation purposes. Email addresses will be used solely to verify eligibility and prevent duplicate responses.
Personal identifiers will be seperated from survey responses as soon as validation checks are complete. Email addresses will not be used in any analysis or reporting and will be deleted following the validation process.

Why your views matter
Your input is invaluable in helping us to improve our processes and approach for the future to ensure we meet the needs of all patients and families affected by serious adverse incidents. The information you provide will be used internally for learning and improvement purposes only.
Respondents are asked to rank a range of statements on a scale of: strongly agree, agree, undecided, disagree, strongly disagree, and not relevant to my situation. A comment box is also provided for some free-text questions.
