Dysphagia Incident Trigger List Survey

This survey is funded by the Public Health Agency and is part of the Dysphagia Ni project.  The project is a partnership made up of stakeholders involved in dysphagia care in Northern Ireland (i.e. from Health and Social Care Trusts, Health and Social Care Board, Public Health Agency, voluntary services and dysphagia service users).  It was set up to drive change across dysphagia services in Northern Ireland.   Dysphagia Ni is involved in the planning, organisation, analysis and reporting of the survey.  The Public Health Agency has been appointed to administer the survey.

Why your views matter

The aim of this questionnaire is to seek the views of healthcare professionals, service users and their carers.   This information will be used to develop the initial draft of the Dysphagia Incident Trigger List prior to further testing.  Thank you for taking the time to consider the document and provide your insights to make it better.
