We are collecting this information to help ensure the quality of the Sage and Thyme training provided to you and others is of the highest standard.

As previously stated, your anonymity will be retained through-out this process. Therefore, you will not be identified in any reports written by our Service for the Trust or in any materials used to publicise the training.

We only collect the information we need to. This will include the reasons you chose to access training, the extent to which it met your expectations, as well as its impact on your perceived ability to handle emotional distress in your future practice. There is also a short section about your experience of accessing training remotely via Zoom.

We will retain your information in line with specific guidance issued by the Department of Health in Northern Ireland. Your response to this survey will be retained for one year, after which it will be deleted.

Please note, before submitting your completed survey, Citizen Space will offer you the opportunity to provide your email address in order to send a copy of your responses to you. Doing so is entirely optional. Should you provide your email address, it will NOT be used as a personal identifier. This ensures your responses will remain anonymous.
